Prescription Refill Request

In our ongoing effort to ensure your pet's health care is as convenient as possible, medication refills can be requested by submitting the following form. This form will be forwarded to a doctor for approval; if the doctor determines the prescription should not be refilled, or deems other medical procedures may be appropriate for your pet before refilling the medication, we will phone you at the number you provide.

We will notify you via phone when your pet's prescription is ready for you to pick up at the hospital. If you have not received a phone call regarding your prescription, contact the office to verify we received your request.

PLEASE NOTE: A physical exam of your pet is required within one year of refilling any medication.

This service is not available for new prescriptions or non-clients.

Please allow ONE business day to process your request. Requests submitted after 4pm on Friday will be processed Monday morning.
